CANCELLATION POLICY

Our office charges a 30% cancellation fee for cancellation within 48 hours of your appointment. No show appointments forfeit full cost of appointment and will be charged with card on file. For medical weight loss clients, appointments cancelled within 48 hours follows our office cancellation policy. Patients that are not cleared by the provider to start our program will receive a refund of $49 for the insurance option and $99 to $199 for the compound weight loss option. Phentermine patients receive a refund if not cleared to start our program at $49. Patients who are cleared and elect to not start our program will be offered a 30 day credit toward another service offered or package. No refunds for prescription orders placed to the pharmacy.
